Program Summary

The primary purpose of the Mental Health Program is to meet the unmet mental health needs of youth and families who are traditionally unwilling to pursue treatment, and whose problems place them at significant risk, and impede adequate functioning within their family, school, community, and mainstream society.

The Mental Health Case Manager provides screening/assessment, intake, case management and service linkage, and wellness groups.

Due to the clientele we serve, stigma is very big. Fears, labeling, and costs are obstacles that get in the way of most of our clients and their families struggle to receive the services they need. Horizons is a one-stop shop! They are able to work through their problems and include their parents to build on their strengths.
